Description

This position is responsible for development, implementation and evaluation of the clinical plan of care for terminally ill patients and their families for the effective and efficient use of all resources in carrying out the organization’s mission.

 

Core Responsibilities Include:

1.    Develops, assesses and delivers the clinical plan of care for terminally ill patients and their families in coordination with interdisciplinary team members

2.    Evaluates patient services provided by the interdisciplinary team

3.    Maintains a knowledge of state, federal and accreditation requirements

4.    Monitors services and documentation to meet policies and procedures, regulations and accreditation guidelines

5.    Serves as liaison between the patient, family, primary physician, Hospice Medical Director and Chief Clinical Officer

6.    Provides direct patient services as defined in the patient’s plan of care

7.    Educates the patient and family on the disease process and proper use of medication and medical equipment

8.    Attends Interdisciplinary team meetings

9.    Initiates patient care conferences as needed

10. Supervises HHA’s and LPN’s as required

Requirements

1.    Licensed in the State of Florida as a Registered Nurse, CHPN preferred.

2.    Valid Florida Driver’s License

3.    A minimum of two years of experience as an RN with background in Hospice, Surgery, Critical Care, Nursing Home or Home Health preferred.

4.    Excellent organizational, communication and interpersonal skills

5.    Leadership ability demonstrated by previous accomplishment

6.    Skills in making physical assessments.

7.    Ability to accept various individual lifestyles.
